Düden Waterfalls

Düden Waterfalls are a group of waterfalls in the province of Antalya. The waterfalls, formed by the recycle station water, are located 12 kilometres northeast of Antalya. They end where the waters of the Lower Düden Falls drop off a rocky cliff directly into the Mediterranean Sea.

1. Introduction

In recent years, with the increasing trend and desire for whiter teeth to enhance aesthetic appearance, teeth whitening is becoming a popular choice of treatment among patients.  The resultant tooth discoloration with or without malformation could compromise the individual appearance, social integration  and quality of life. Hence, the treatment of discolored teeth has gained prominence in the practice of esthetic dentistry.

The lightening of the tooth color  through the application of a chemical agent to oxidize the organic pigmentation in the tooth is referred to as bleaching. Bleaching treatment is considered to be the most conservative, safe, effective, and minimally invasive method to overcome external and internal tooth discoloration to esthetically alter the color of teeth.

The advantage of teeth whitening lies in those treatments in which the dental structures are preserved. In addition, with the advent of new bleaching agents, the procedure is relatively quick and safe.

2. Tooth Discoloration

The etiology, appearance, localization and severity of discolored teeth vary, and the discoloration is classified as intrinsic, extrinsic, or both according to its location and etiology. Extrinsic discoloration is caused by the habitual intake of chromogenic dietary sources such as, wine, coffee, tea, carrots, oranges, chocolate, tobacco, mouth rinses, or plaque on the tooth surface. As for intrinsic discoloration, it can either be caused by systemic or local factors. Systemic causes include drug-related (tetracycline), metabolic, fluorosis, and genetic (hyperbilirubinemia, amelogenesis imperfecta, and dentinogenesis imperfecta). Local causes include pulp necrosis, intrapulpal hemorrhage, pulp tissue remnants after endodontic therapy, endodontic materials, coronal filling materials, root resorption, and aging.

3. Teeth Whitening Procedures

Few techniques in dentistry are as broadly applicable as teeth whitening. The indications for teeth whitening are direct patient desire for teeth whitening and/or restorative treatment needs involving the overtly visible smile. First, the patient has to desire whiter teeth and a more uniform tooth color. The second indication involves patients with a treatment plan that includes restorative or esthetic dentistry involving the visible dentition. While there are few contraindications, special consideration should be applied in cases of sensitivity, pediatrics, or pregnancy. Existing tooth sensitivity is a potential contraindication.

Teeth exhibiting yellow or orange intrinsic discoloration seem to respond best to vital bleaching. Brown fluorosis stains also are often responsive to treatment, but White fluorosis stains may not be resolved efectively (although they can be made less obvious if the surrounding tooth structure can be significantly whitened).

Depending on the concentration of the bleaching agent, the product will have different indications. In vital teeth, there are different whitening guidelines: clinical, at home and combined treatment.

3.1 In-Office Bleaching

Bleaching of vital teeth can be done by in-office method, dentist-supervised home bleaching or self-bleaching using over-the-counter products. An in-office bleaching, hydrogen peroxide (concentrations varying from 15% to 38%) is directly applied on the tooth surface. To accelerate the bleaching process, the bleaching agent may be “photo” activated using light-emitting diodes (LED), plasma arc, quartz halogen, ultraviolet units, and lasers. Referred to as “power bleaching.” Application of light-activation method leads to a decrease in treatment time and reduction in adverse effects of bleaching materials. LED applications are usually performed in three to four sessions at seven-day intervals.

  • The advantages of the in-office vital bleaching technique are that (although it uses very aggressive chemicals) it is totally under the dentist’s control, soft tissue is generally protected from the process, and the technique has the potential for bleaching teeth more rapidly.
  • Disadvantages primarily relate to the cost, the unpredictable outcome, and the unknown duration of the treatment. The features that warrant concern and caution include the potential for soft tissue damage to both patient and provider, discomfort caused by the rubber dam or other isolation devices, and the potential for posttreatment sensitivity.

3.2 Home Bleaching

Professionally prescribed home bleaching agent has been proven to be efficient to whiten discolored teeth. Home bleaching is closely monitored by the dentist, thereby ensuring their safety and increasing the effectiveness of the bleaching procedure.

The ‘home vital bleaching’ treatment typically employs carbamide peroxide at concentrations of 10 to 16%, which represents low concentrations of hydrogen peroxide but is performed for a much longer time than in-office whitening, applied in individual trays used at night (nightguard), with four-eight hour applications, or during the day, for a period of two to five weeks.

  • The advantages of the dentist-prescribed, home-applied technique are the use of a lower concentration of peroxide, ease of application, minimal side efects, and lower cost because of the reduced chair time required for treatment.
  • The disadvantages are reliance on patient compliance, longer treatment time, and the (unknown) potential for soft tissue changes with excessively extended use.

NOTE: To obtain results that are less aggressive to the dental tissues and more durable with respect to the longevity of the bleaching, the current trend is to use a combination of techniques in-office and at-home. It has been shown that patients prefer those therapies which last less time, and combined treatment need more time than the clinical one.

3.3 Walking Bleaching

This technique provided a dependable treatment modality for teeth whitening, but its use was obviously limited to endodontically treated teeth. 30%-35% hydrogen peroxide and sodium perborate either in combination or separately are the most commonly used agents for non-vital bleaching of endodontically treated teeth, in which oxidation reaction and degradation of pigment molecules are resulted.

4. Side Effects And Long Term Stability Of Teeth Whitening

Despite the advantages offered by vital teeth whitening, adverse side effects such as tooth sensitivity, changes in surface roughness, decreases in microhardness, changes in surface morphology and changes in the chemical composition are described in the literature.

Because hydrogen peroxide has such a low molecular weight, it easily passes through enamel and dentin. This characteristic is thought to account for the mild tooth sensitivity occasionally experienced during treatment. Patients may experience transient tooth sensitivity or irritated gingiva between appointments, but no long-term adverse efects of bleaching teeth with otherwise healthy pulps have been reported in the literature.

An educated avoidance (or decreased consumption) of chromogenic foods, particularly during and immediately after teeth whitening, can delay the inevitable color relapse. A meticulous regimen of oral hygiene on the patient’s part  also assists in maintaining the whiteness of the teeth for a longer period. Improved home care is often a noticeable and beneficial consequence of whitening procedures, as the patient becomes more personally aware of the benefits of a healthy smile.

Bleaching generally has an approximate lifespan of 1 to 3 years, although the change may be permanent in some situations.

Tooth sensitivity is a common dental problem that involves discomfort or pain in teeth when encountering certain substances and temperatures. The pain caused by hot or cold foods could be a sign of a cavity; it’s also common in people who have sensitive teeth. The pain is often sharp and sudden, but it is temporary. Fortunately, sensitive teeth can be treated and the condition can improve.

What causes to Tooth Sensitivity?

In healthy teeth, enamel protects the underlying layer of dentin, which is softer than enamel. Pain and discomfort occur when the dentin, the layer beneath your tooth enamel, is exposed. Dentin is connected to your nerves, so when it comes in contact with certain triggers, you experience pain. 

Some people naturally have more sensitive teeth than others due to having thinner enamel. The enamel is the outer layer of the tooth that protects it. In many cases, the tooth’s enamel can be worn down from:

  • Brushing your teeth too hard, causing your enamel to wear down;
  • Receding gums and gingivitis, which leads to exposure of your roots and dentin;
  • Cracked or broken teeth, which cause inflammation and even infection that, can trigger sensitivity;

How to Treat Tooth Sensitivity?

There are options for resolving tooth sensitivity. Proper diagnosis of the reason for the sensitivity is essential in treating sensitivity. If the reason for the sensitivity is addressed, the treatment chosen will be more successful in decreasing pain.

Dietary changes

Eliminating tooth sensitivity may require dietary changes. You can start by avoiding highly acidic foods, which can weaken tooth enamel.

Healthier brushing habits

Switch from a hard-bristled toothbrush to a soft-bristled toothbrush, and don’t brush too vigorously. You can choose toothpaste that’s labeled as being specifically made for sensitive teeth. These toothpastes won’t have any irritating ingredients, and have desensitizing ingredients that help block the discomfort from traveling to the nerve of the tooth. When it comes to mouthwash, choose an alcohol-free mouth rinse, as it will be less irritating to sensitive teeth.

Dental procedures

If the desensitizing toothpaste does not decrease your discomfort, your dentist may suggest in- office treatments. A fluoride gel or special desensitizing agents may be applied to the sensitive areas of the affected teeth.   • Bonding crowns or inlays. These may fix a tooth flaw or decay that is causing sensitivity.
  • Fluoride gel or varnish.
  • Surgical gum graft. This will protect the root and reduce sensitivity if the gum tissue has eroded from the root.
  • Root canal. This is a last-resort treatment for severe tooth sensitivity that has not been helped by other methods.

For long-term solutions, you need to speak with a dental professional to determine the underlying cause and best treatment plan. Veneer and Crown: What’s the difference?

Veneers and crowns are long-lasting solutions for patients who suffer from some functional and cosmetic dental problems and are looking to bring back their smiles. Dull, discolored or misshapen teeth, gaps between teeth, and broken teeth can be addressed with veneers and/or crowns.

Veneers and crowns are both dental restoration methods that can improve the look and function of your teeth. The main difference is that a veneer covers only the front of your tooth and a crown covers the entire tooth. Here’s a look at the differences between veneers and crowns, the pros and cons of each, and how they’re used.

What is a Veneer?

A veneer covers only the front surface of your tooth. Unlike crowns, veneers are considered a conservative solution to a compromised smile because almost all of the original tooth is left intact. When veneers are chosen, half a millimeter of enamel are removed to allow for the porcelain veneer to be placed on the remaining enamel.

E.max porcelain or lithium disilicate glass-ceramic porcelain can be used to make laminate veneers. E.max laminate veneer is the most commonly applied laminate veneer type in our dental centre in Turkey.

What’s involved with getting a Veneer?

The dentist will make an impression of your prepared tooth by using a mold. After impressions will be sent to the dental lab. Depending on how much your tooth was trimmed, you may have a temporary veneer placed on the tooth until the new one is ready.

3 days later, the permanent veneers will ready for check the function, aesthetic and harmony. If correction is required, it is sent back to the laboratory. Then if everything is okey, veneers are bonded. Light-sensitive resin is applied between the original tooth and the veneer. A special light is then trained on the area to cure or harden the resin creating an irreversible bond between tooth and veneer.

There’s typically minimal movement of the tooth after the veneer is in place. But you may need to wear a night guard to protect the veneer if you grind or clench your teeth at night.

What is a crown?

Crowns are considered a more aggressive solution than veneers. Usually crowns are recommended for the teeth that are badly broken or cracked, or where root canal treatment has been needed. Unlike veneers that are thin porcelain layers that cover the front of the tooth and the biting edge, a crown is fabricated of thicker porcelain and completely covers the entire tooth right down to the gum line.

If you have tooth decay, your dentist will remove the decayed part of the tooth before making the crown. After the filling is made, crown preparation is made.

At Dental Excellence Turkey, we use metal-supported porcelain crowns, zirconium crowns, E.max crowns, ZirCAD Prime crowns. ZirCAD Prime crown , E.max crown and Zirconium crown are more preferred because they have aesthetically superior properties than metal-supported porcelain crown.

What’s involved with getting a crown?

Your dentist will produce an impression of your tooth by making a mold. The mold will be sent to the dental lab for fabrication of the crown.

The dentist may place a temporary crown on your ground-down tooth so that you can use your tooth while the permanent crown is being made.

When the permanent crown is ready, the dentist will remove the temporary crown. They’ll then place the permanent crown on your tooth and will adjust it so that it fits correctly and your bite is right. Then they’ll cement the new crown into place.

Teeth with crowns may have some movement, which can change your bite. If this happens, you’ll need to have the crown adjusted.

Veneers vs. Crowns: Which One Is Right for You?

A veneer is a very thin layer of porcelain or other materials, about 1 millimeter (mm) in thickness, that’s bonded to the front of your existing tooth. Laminate veneers are used mainly for cosmetic enhancement.

A crown is about 2 mm in thickness and covers the whole tooth. If you have lost 50%-60% of your tooth structure because of decay, trauma or other reasons, we choose the crowns for your treatment.  As we know that the enamel is the strongest matter in the body, and we know that once this enamel is damaged or lost, it means that the remaining part is the most breakable part of the tooth. Dental crown replaces all of the lost enamel. So, it is like replacing the tooth itself.

Pros of getting a Crown done:

  • Strong and long-lasting
  • Can be budget-friendly as they are made up of various materials (Porcelain Fused to a Metal Alloy (PFM), Zirconia or Emax)
  • Protection from sensitivity.
  • After the root-canal procedure, it becomes the main support for the remaining tooth structure.
  • No need to worry about chewing forces when on crowns.
  • Any tooth (anterior/posterior) can be restored with a crown.
  • If the crown falls, it can be re-fixed without the need for replacement.

Cons of getting a crown done:

  • Tooth-preparation is needed.

Pros of getting dental veneer done:

  • Without extensive orthodontic treatment looks fantastic.
  • Gives an option for getting an instant smile makeover.
  • They need minimal tooth preparation.
  • Gives the perfect illusion of natural teeth.

Cons of getting veneer done:

  • Veneers are prone to chipping, especially if the person has bad habits like grinding teeth or biting hard things
  • Also it is harder for dentists to do things like contact veneers because they are very difficult to handle and fix to the teeth.
  • Requires experience dentists and technicians to create these small pieces of porcelain.
  • Hence, veneers are more expensive than crowns.
